VB Appliation on Extended Monitors

I have one VB + SQL application which is running on Client computer.
Client wants to show some information to end customer on differnt form and this form should get displayed on extended monitor automatically.
For e.g. My software name is HHH and form name is XYZ. some information of XYZ should get displayed on PQR form (same time - realtime) which has to be opened on extended monitor.
Extended Monitor - One extra monitor is connected to Computer with Graphics Card.

does any one knows the solution or archetecture of such solution?

i am worried -
1.how i can show form XYZ on extended monitor each time when Client is switching off computer daily.
2.how i will show XYZ form info on PQR form same time as - XYZ data is changing frequently
