pls help!!!! search between two dates plus to fields

good day! im newbies in vb 6.0, im creating a project that will filter my tables between two date plus two different fields in my database.
i manage to make a query but unfortunately it seems that it cannot support the filtering that i want.
i attached my source codes.
i will be very thankfull in any guidance or any possible help to my problem!
